GUARANTEE
AND REFUND POLICY
FamilyDentalUSA will issue a full refund of the membership fees
you have paid within the first thirty days
from the purchase date of the GE Wellness Plan without question. Refund requests that are made
after the first 30 days will be prorated over the remaining period minus a $20 administrative and processing fee.
All cancellations must be submitted to FamilyDentalUSA in writing by US mail, fax
or e-mail. Please contact FamilyDentalUSA toll free at 1-866-DENTAL5
for further information.
GE Wellness Plan and FamilyDentalUSA make every effort to ensure,
but cannot guarantee, the accuracy
and completeness of the information contained in the participating
provider directory. However, from time to time, providers
may withdraw from participation in the GE Wellness Plan provider network.
The provider directories
are available only and solely as a means for members to determine
which providers may currently be participating in the GE Wellness
Plan.
The GE Wellness Plan Customer Service staff are available to
assist you in finding a participating dentist or vision provider.

site shall be the membership fee you paid. In no event

or punitive damages related to membership in the GE Wellness

Order Acceptance Policy
Your receipt of an electronic or other form of order confirmation
does not signify our acceptance of your order, nor does it constitute
confirmation of our offer to sell. FamilyDentalUSA and GE
Wellness Plan reserve the
right at any time after receipt of your order to accept or decline
your order for any reason. Additionally, FamilyDentalUSA may require
additional verifications or information before accepting any order.
FamilyDentalUSA HEREBY GIVES NOTICE THAT, UNLESS YOU CANCEL YOUR
MEMBERSHIP, YOUR MEMBERSHIP WILL
BE AUTOMATICALLY RENEWED TWO (2) WEEKS PRIOR TO THE EXPIRATION
DATE IDENTIFIED IN YOUR ORDER CONFIRMATION.
Unless otherwise advised, the automatic renewal will be charged
to the credit card account that you provide in making your initial
membership order. In the event your initial membership order is
paid by personal check, it is the responsibility of the member
to supply FamilyDentalUSA with your current valid credit card
account information or a personal check for the purposes of payment
of membership renewal. In order for your membership to continue
without interruption, your renewal payment must be received two
(2) weeks prior to the expiration date identified in your order
confirmation. In the event that FamilyDentalUSA attempts to automatically
renew your membership but is unable to do so due to the cancellation
or inactivation of your credit card account, FamilyDentalUSA will
attempt to notify you through the e-mail address on record
and inform you that your membership will expire on the
date identified in the order confirmation. In all events, it is
your responsibility to ensure that payment for membership
renewal has been received by FamilyDentalUSA two (2) weeks prior
to the expiration date identified in the order confirmation. FamilyDentalUSA
is not responsible and assumes no liability to you for your failure to provide FamilyDentalUSA with current valid
credit card account information or other payment for membership
renewal.
BY ELECTRONICALLY ACCEPTING THE TERMS AND CONDITIONS OF THE FamilyDentalUSA
ORDER ACCEPTANCE POLICY, YOU AGREE AND UNDERSTAND THAT YOU ARE ENTERING INTO A BINDING AND ENFORCEABLE LEGAL CONTRACT.
Jurisdiction & Venue
You agree that any legal action brought against FamilyDentalUSA,
LLC, shall be governed by the laws of the State of California
without regard to its conflict of law principles and that any
legal action brought against Signature Agency, Inc., will be governed
by the laws of the State of Illinois without regard to its
conflict of law principles.
